An intake worker is available to set you up with an appointment and answer any questions you may have about the counseling process. When calling to arrange an appointment, please have the following information available:
- The reason you are seeking counseling. (This information will help us determine which therapist will best fit your needs)
- Any insurance information. (This information will help determine any of your financial responsibilities for counseling).
To Make Your First Appointment Easier
Please feel free to print and complete the folloiwng documents and bring them to your first appointment. This will allow you to review our paperwork and complete your information before you arrive for your first appointment. Completing paperwork will allow you to come 10 minutes before your first appointment instead of the usual 20 - 30 minutes.
